In our vast experience the weak link can often be with the support to which the personnel who are at the sharp end need. Either they are doing necessary tasks themselves often poorly, there is a lack of a coherent strategy with objectives and measurement/accountability. Also there is a tendancy to delegate sales support to an existing member of staff, usually untrained in this area
